What the service looks like.
During the follow-up psychiatric assessment, you will:
1. Fill out two questionnaires: a basic medical history questionnaire to update patient records and psychometric measures to assist clinicians better understand your illness and symptoms.
2. Consult a psychiatrist who will:
• Review your medical history, examinations, and treatment plans.
• Conduct a complete mental health evaluation.
• Review your concerns, symptoms, and progress since your last visit.
• Educate patients on prescription refills and maintenance, and make necessary adjustments as needed.
• Create a personalised treatment plan to meet your specific needs.
What To Expect
Prior to your meeting, you will be emailed a link to the relevant questionnaires. Please allow 24 to 48 hours for the team to examine your responses before the booked appointment.
During your consultation, our psychiatrist will carefully listen to your problems, answer all of your questions, and collaborate with you to develop a comprehensive treatment plan. You will also receive medicine refills when your current supply runs out, ensuring that you can continue with your therapy and efficiently manage your symptoms.
